Below you will find the links to the registration forms needed to attend the El Paso Southwest Youth Conference.
REMINDER: Any adult persons attending or volunteering at the conference must have completed and be up to date with their Virtus Training. If you are unsure about your update status, please contact your Safe Environment Coordinator.
STEP 1- Download this Registration Checklist Form this form will help you make sure you have all forms you will need for your entire group.
STEP 2- If you are taking a group, download this Group Leader Form to keep track of all those attending in your group.
STEP 3- Download this Individual Registration Form for every person attending, NOTE: If you are Volunteering please make sure to fill out the Volunteer

STEP 4- Download this Chaperone Guideline Form for every Chaperone attending, NOTE: All Chaperones must be up to date with their Virtus. Contact your parish Virtus Coordinator to ensure everyone on your list is up to date. Please ensure that for every 8 teens that you have 1 adult chaperone.
STEP 5- WE LOVE OUR PRIESTS AND IT MEANS THE WORLD TO SEE PRIESTS
SO INVOLVED AND SUPPORTIVE OF OUR YOUTH.
If you have invited your priest, download this Priest Registration Form for every priest attending. Priests do not have to pay a registration fee but we encourage and welcome them to assist in Reconciliation and to participate in the Celebration of the Holy Mass on Saturday.
STEP 6- WE CAN'T MAKE THIS CONFERENCE HAPPEN WITHOUT VOLUNTEERS.

What forms do you need?
If you are bringing a group please fill out the following:
-Group Leader Form
- Individual Registration Form (for every individual attending the conference including the group leader)
- Priest Registration Form (Please make every effort to have a priest present)
- Chaperone Guideline Form
If you are an individual please fill out the following (25 years of age or older):
- Individual Registration Form
If you are an individual please fill out the following (younger than 25 years of age):
- You MUST be accompanied by a chaperone
- Individual Registration Form ( for both yourself and the chaperone)
